Agreement:

I have voluntarily agreed to participate in a product research project for Blade Labs Inc. and its affiliated companies. (“Blade Labs”). 

I authorize Blade Labs representatives to capture my comments, opinions, suggestions and ideas for improving or otherwise modifying Blade Labs’ products or services (all of which are referred to as “Feedback”). I understand and agree that Feedback may include any of the following, whether provided during or after the session:

• Spoken statements
• Written statements, including drawings made by me

My Feedback may be captured by Blade Labs in any of the following formats:

• Audio recordings
• Video recordings and photographs
• Written notes, including drawings 
  
  

I waive my rights to confidentiality and any ownership rights in Feedback. Nothing in this Agreement or in the parties’ dealings arising out of or related to this Agreement will restrict Blade Labs’ right to use, profit from, disclose, publish, keep secret, or otherwise exploit Feedback without compensating or crediting me for providing such Feedback. 

I understand that I may be exposed to confidential information, including current and future products, product capabilities, and user interfaces; and current and future processes and services. Much of this information is not generally known or available to the public, and it may have substantial commercial value to Blade Labs. 

I understand that I should assume that whatever I learn as a result of my participation in this project is confidential and proprietary to Blade Labs. Therefore, I agree to keep all of the confidential information disclosed to me in the strictest confidence and agree that I will not use the information or disclose it to anyone without Blade Labs’ prior written approval. However, I understand that this paragraph doesn’t apply to anything Blade Labs’ has published (e.g., on its website) and that in any event I will be released from this obligation three years from the date of my signature below. 

I understand that my personal information will be processed by Blade Labs in compliance with all relevant privacy and data protection laws and Blade Labs’ Privacy Policy.

Acknowledgement and Agreement: By submitting this form, I acknowledge that I have read this form, have had the opportunity to ask questions about the research, have received satisfactory answers, and that I am able to and want to participate on the terms listed in this form.
